- No, in the majority of cases, you do not need a doctor referral to be seen. Indiana is a direct access state meaning physical therapists can see a patient without doctor referralfor the first 42 calendar days.
- Some insurances, including Medicare, do require a physician to be following your plan of care and willing to sign off on the physical therapy evaluation.

The only insurance we will is Medicare at this time. All other patients will be cash pay.
However, we provide receipts that contain all of the information insurances require for submission to cover out-of-network physical therapy services. Therefore, we have had clients that have submitted our receipts to their insurances and they have received reimbursement for our services. Of course, every insurance is different and out-of-network coverages vary depending on your specific plan.

We do see Medicare patients for physical therapy as we are an official Medicare provider.

We charge $60 to come to your home for a 30-minute visit, $90 for a 45-minute visit, or $120 for an hour-long session.
- The above costs are assuming you are located within 20 minutes of our headquarters in Westfield, IN. If you are located outside that area and it would take 21-30 minutes to travel to you, a $20 travel fee is added on, but in many cases, this can be waived.
- If you live further than 30 minutes outside the area, we can still probably see you, but you would need to contact us to determine what the fee would be for travel.

We have very low overhead. By traveling to see you, we don’t worry about the costs of owning or renting a building and we don’t need any accessory staff/employees.

We see patients Friday through Sunday from 8:00am to 6:00pm. However, we will sometimes see patients outside of these hours and after work hours during the week in order to help people be seen.

Mobile Wellness Physical Therapy visits may be treated as “out-of-network” physical therapy treatments by insurance plans that offer those benefits. Our providers will give you an invoice (or super bill) that can be submitted to your insurance with all of the necessary information (license numbers, tax ID, business name, etc.) as well as your specific diagnosis and billing codes. You may choose to submit bills after each visit or any other frequency, typically up to one year following the provided treatment visit.
While we cannot guarantee reimbursement by your insurance provider, we will provide you with the resources necessary to request reimbursement for our services.
